Your Responsibilities

If your insurance requires pre-certification, pre-authorization or other approvals, it is your responsibility to obtain them.  Please be aware that many insurance companies do not guarantee payment even if they have authorized the services.  You are responsible for paying all charges not covered by insurance.  This may include but is not limited to deductibles, co-pay amount, and non-covered charges.  You may be required to pay for any unpaid health care items or services that your insurance plan deems non-covered or not medically necessary.

Filing a Claim


As a convenience to our patients, Cardinal Vision LLC will file a claim for your healthcare provided we have complete and accurate billing information and benefits are made payable to Cardinal Vision LLC.  If we are unsuccessful in obtaining payment from your insurance company, we may request your assistance to resolve the issue. 



Payment of the good faith estimate is due at the time of the exam.  We accept checks and most major credit cards.  Credits and refunds will be applied for any overestimation of the patient's responsibilities after payment from the insurance company has been processed.
