All patients should arrive ~15 minutes early in order to complete the registration process.

• Drivers License or Government Photo ID

• Patient's Insurance Card(s)

• Physician Referral Form (if required by insurance)

• Means of Payment

• Any Eye Drops you are currently using.

Cardinal Vision LLC is a provider for Medicare and most major insurance plans.  We provide insurance billing. Please be aware that any costs not covered by your insurance are your responsibility and will be collected at the time of your appointment.
